  • NHS Number (if known)
  • Postcode
  • Which medication you are requesting
  • Which Pharmacy you would like the Prescription sent to

Pharmacy

Arrange with a local or online pharmacy to request and collect/deliver your prescription.

Other Information

Please note that we do not take prescription requests verbally or via fax because errors are easily made with this method.

Electronic Prescription Service (EPS)

The Electronic Prescription Service (EPS) is an NHS service. It gives you the chance to change how your GP sends your prescription to the place you choose to get your medicines or appliances from.

Repeat Dispensing

Introduced in 2006 was a Repeat Dispensing Scheme. This allowed patients having regular medication to have their prescriptions issued for 6 or 12 months (where appropriate). This is done in conjunction with Local Pharmacies and may mean you do not have to visit the surgery everytime you need a repeat prescription.

Medication Reviews

We have a duty to review your health and medicines regularly. We will let you know if you need to make an appointment for a medication review with either a Practice Pharmacist, Nurse, or your doctor.

If you need urgent advice about your medication outside our surgery hours, please call NHS 111.
